How do we write test plans?

Each qualification plan shall have one or more test plans, written in a way that it is possible to see
• What shall be tested
• How it shall be tested
• Expected results for the testing

Often the test plans have open fields that can be filled in during the testing with reference to documentation and/or shorter comments. Needless to say, the documentation shall of course be dated and signed by the tester, and approved/authorized by someone else.
